Introduction

Forms is an Office 365 app that allows you to quickly and easily create custom quizzes, surveys, questionnaires, registrations and more. When you create a form, you can invite others to respond to it using any web browser, even on mobile devices. As results are submitted, you can use built-in analytics to evaluate responses. Form data, such as quiz results, can be easily exported to Excel for additional analysis.

Important Note

In accordance with the Queen's Policy on Administrative Student Surveys:

"all administrative student surveys are expected to utilize Queen's survey software (currently Qualtrics) unless the researcher(s) and Institutional Research & Planning agree that special circumstances warrant use of another system."

In addition, users acknowledge and agree that they will not use Forms to conduct surveys that put Queen's in violation of FIPPA, PHIPA and/or PIPEDA, or collect other personal or confidential information without appropriate approval. Eligibility

Forms is part of the Office 365 suite of cloud-based services available to all current students, staff and faculty of Queen's University with an active Queen's NetID@queensu.ca email account.

Alumni and retired staff and faculty are not eligible for Forms.

Getting Started

Forms can be accessed from the apps launcher in the Office 365 Portal, https://office.com/signin or from https://forms.office.com.
