Introduction 

Forms is an Office 365 app that allows you to quickly create custom quizzes, surveys, questionnaires, registrations, and more. When you create a form, you can invite others to respond to it using any web browser or Microsoft Teams, even on mobile devices. As results are submitted, you can use built-in analytics to evaluate responses. Form data, such as quiz results, can be easily exported to Excel for additional analysis.

"All administrative student surveys are expected to utilize Queen's survey software (currently Qualtrics) unless the researcher(s) and Institutional Research & Planning agree that special circumstances warrant use of another system."

In addition, users acknowledge and agree that they will not use Forms to conduct surveys that put Queen's in violation of FIPPA, PHIPA and/or PIPEDA, or collect other personal or confidential information without appropriate approval. Eligibility

Queen’s students, faculty, and staff with an active NetID@queensu.ca are eligible to use Forms at no cost. Alumni and retired staff and faculty are not eligible for this service. 

Getting Started

Users can access Forms at https://forms.office.com  or from the apps launcher in the Office 365 Portal. 

Forms is designed to work with the current or immediately previous versions of Internet Explorer or Firefox, or the latest version of Chrome or Safari. For more information, see Microsoft 365 system requirements.
